患者男,65岁,患高血压病10年,因发作性胸闷心慌2月于1993年5月3日入院.以往常服复方降压片、硝苯地平等,未用过丹参制剂,无哮喘病史.体检:心肺正常,BP 24/14kPa;ECG:窦性心律,电轴左偏,左心室肥大.入院后给予复方丹参液(上海第九制药厂,批号930322123)20mL加5%葡萄糖500mL静滴.约3h后出现鼻塞、咽痒、打喷涕、胸闷心慌、呼吸困难、端坐位、喘息貌、口唇紫绀,双肺布满哮鸣音,HR110次/min,律整.给予吸氧,静注地塞米松10mg及10%葡萄糖酸钙10mL,肌注异丙嗪25mg.随后症状逐渐缓解.次日在严密观察上再次静滴上述液体,约半小时又出现上述哮喘发作症状,同时全身皮肤出现荨麻疹,经紧急处理后哮喘缓解.第3天改用川穹嗪120mg加5%葡萄液静滴,未见症状再发生,半月后病情稳定出院.
