目的探讨3D-CTA减影技术及2D-DSA在颅内动脉瘤诊治中的价值。方法回顾性分析38例动脉瘤患者3D-CTA、2D-DSA和手术资料。CTA与DSA图像相比较观察动脉瘤位置、大小、形态、毗邻关系、穿支动脉等。结果 38例患者手术加介入证实有动脉瘤46枚,3D-CTA检出动脉瘤44枚,2D-DSA检出动脉瘤42枚,两者均有遗漏。3D-CTA准确率96%,敏感性为96%,特异性为100%;2D-DSA准确率91%,敏感性97%,特异性93%。3D-CTA可准确清楚地显示动脉瘤发育方向、毗邻关系能够成功构建其三维形态及空间关系。结论 3D-CTA是检查颅内动脉瘤高度敏感的方便快捷、无创、影像学手段,其诊断效价优于2D-DSA,可以作为动脉瘤诊断首选。
Objective To investigate the value of 3D-CTA subtraction and 2D-DSA in the diagnosis and treatment of intracranial aneurysms. Methods The 3D-CTA, 2D-DSA and surgical data of 38 patients with aneurysm were retrospectively analyzed. CTA and DSA images were compared to observe the location of aneurysms, size, morphology, adjacent relationship, perforation of arteries. Results A total of 46 aneurysms were confirmed by surgery and intervention in 38 patients, 44 aneurysms were detected by 3D-CTA, and 42 aneurysms were detected by 2D-DSA. The accuracy of 3D-CTA was 96%, the sensitivity was 96% and the specificity was 100%. The accuracy of 2D-DSA was 91%, the sensitivity was 97% and the specificity was 93%. 3D-CTA can accurately and clearly show the aneurysm development direction, adjacent to the successful construction of its three-dimensional morphology and spatial relationship. Conclusion 3D-CTA is a convenient, non-invasive and imaging method for detecting intracranial aneurysms. Its diagnostic efficiency is superior to that of 2D-DSA, which can be the first choice for the diagnosis of aneurysms.
