目的:探讨孕妇妊娠期高血压疾病对新生儿脐血脂联素水平的影响以及脂联素与围产儿生长发育之间的关系。方法:选取2011年4月~2012年4月间该院产科患有妊娠期高血压疾病的孕妇分娩出的新生儿57例,按孕妇状态分为妊娠期高血压组22例、子痫前期轻度组21例、子痫前期重度组14例。选取同期在该院进行常规体检的正常孕妇57例作为对照组。对比各组间新生儿的脐血脂联素水平以及围产儿生长发育情况。结果:同对照组相比,妊娠期高血压疾病组新生儿脐血脂联素水平明显偏低,随着孕妇妊娠期高血压疾病病程的加重呈逐渐下降趋势,组间比较差异具有统计学意义(P<0.05);妊娠期高血压疾病组新生儿胎龄、体质量指数(BMI)、出生体质量、头围和体长均明显低于对照组,组间比较差异均具有统计学意义(P<0.05);妊娠期高血压疾病组新生儿脐血脂联素水平和新生儿的胎龄、BMI、出生体质量间均呈正相关(P<0.05)。结论:妊娠期高血压疾病对胎儿的内分泌代谢调节功能有严重影响,随着孕妇妊娠期高血压疾病病情的加重而加重;脂联素和胎儿生长发育密切相关,可作为衡量胎儿生长发育的一个有效指标。
Objective: To investigate the effect of gestational hypertension on pregnant women with umbilical cord blood adiponectin levels and the relationship between adiponectin and perinatal growth and development. Methods: From April 2011 to April 2012, 57 pregnant women with gestational hypertension in the obstetric department were enrolled. Fifty-seven newborns were divided into gestational hypertension group (n = 22), preeclampsia 21 cases of degree group and 14 cases of severe preeclampsia group. Select the same period in the hospital for routine physical examination in 57 pregnant women as a control group. The levels of umbilical blood adiponectin in neonates and the growth and development of perinatal infants were compared. Results Compared with the control group, neonatal umbilical blood adiponectin levels in gestational hypertension group was significantly lower, with the progression of hypertensive disorders during pregnancy decreased gradually, the difference between the groups was statistically significant ( P <0.05). Neonatal gestational age, body mass index (BMI), birth weight, head circumference and body length in gestational hypertension group were significantly lower than those in control group, and there were significant differences between groups (P <0.05). There was a positive correlation between neonatal umbilical blood adiponectin levels and neonatal gestational age, BMI and birth weight in gestational hypertension group (P <0.05). CONCLUSION: Hypertensive disorders in pregnancy have a serious impact on fetal endocrine and metabolic regulation, which aggravates with the worsening of pregnancy-induced hypertensive disorder. Adiponectin is closely related to fetal growth and development and may be used as a measure of fetal growth and development Effective indicator.
