目的探讨α2β1整合素基因多态性与糖尿病合并冠心病、血脂异常之间的关系。方法采用PCR-RFLPs法,对217例南京地区2型糖尿病(T2DM)患者[合并冠心病101例(A组),无冠心病116例(B组)]和107例正常对照者(C组)进行整合素α2β1Bg1Ⅱ酶切位点多态性研究,并根据冠状动脉造影结果分组分析。结果 (1)A组Bg1Ⅱ+/+基因型(56%)显著高于B组(23.3%)和C组(39.3%)(P<0.01);Bg1Ⅱ基因阳性率与冠心病冠脉病变支数呈正相关趋势;(2)Bg1Ⅱ+/+型者胰岛素抵抗指数(HOMA-IR)明显高于Bg1Ⅱ+/-及Bg1Ⅱ-/-者(P<0.05),而胰岛素敏感指数(ISI)降低;(3)不同Bg1Ⅱ基因型的各亚组间血浆总胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)水平没有显著性差异(P>0.05)。结论α2β1整合素Bg1Ⅱ酶切位点多态性与冠状动脉病变间存在相关性,可能与T2DM胰岛素抵抗、动脉粥样硬化进展有关。
Objective To investigate the relationship between α2β1 integrin gene polymorphism and diabetes with coronary heart disease and dyslipidemia. Methods 217 cases of type 2 diabetes mellitus (T2DM) [101 cases with coronary heart disease (group A), 116 cases without coronary heart disease (group B)] and 107 normal controls (group C) were analyzed by PCR-RFLPs. Integrin α2β1Bg1 Ⅱ restriction site polymorphism study, and based on coronary angiography results grouped analysis. Results (1) The Bg1Ⅱ + / + genotype (56%) in group A was significantly higher than that in group B (23.3%) and group C (39.3%) (P <0.01) (2) The insulin resistance index (HOMA-IR) of Bg1Ⅱ + / + type was significantly higher than that of Bg1Ⅱ +/- and Bg1Ⅱ - / - (P <0.05), while the insulin sensitivity index (ISI) 3) There were no significant differences in the levels of total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C) between different subgroups of Bg1Ⅱ genotypes Difference (P> 0.05). Conclusion There is a correlation between Bg1 Ⅱ polymorphism of α2β1 integrin and coronary artery lesion, which may be related to the progress of insulin resistance and atherosclerosis in T2DM.
