目的探讨绝经期子宫内膜息肉的发病机制。方法选取30例绝经期子宫内膜息肉患者作为实验组,选取绝经期子宫内膜正常者30例作为对照组。息肉组均进行宫腔镜下子宫内膜息肉电切术,取息肉组织为实验组一,取远离息肉的内膜组织为实验组二。经石蜡病理证实为良性子宫内膜息肉。实验组和对照组均检测外周血激素6项,将两组中雌激素进行对比,应用免疫组织化学方法测定息肉及子宫内膜中芳香化酶的表达,用Image-Pro Plus 6.0图像分析软件进行图片分析,平均光密度值为比较指标。结果实验组和对照组血清雌激素水平分别为:(16.5±2.11)pg/ml和(15.8±2.28)pg/ml,两组外周血雌激素水平差异无统计学意义;实验组一、实验组二和对照组组织中芳香化酶表达的平均光密度分别为(0.101 1±0.001 3)、(0.025 5±0.001 0)、(0.024 7±0.000 7),以α=0.05的检验水准,实验组一中芳香化酶的表达明显高于实验组二,差异有统计学意义(P<0.05);实验组二中芳香化酶的表达与对照组差异无统计学意义(P>0.05)。结论绝经期子宫内膜息肉的形成不依赖于体循环雌激素水平的升高,可能与子宫内膜局部芳香化酶过表达有关。
Objective To investigate the pathogenesis of endometrial polyps in menopause. Methods Thirty patients with endometrial polyps were selected as the experimental group and 30 normal menopause endometrium were selected as the control group. Polyps group were underwent hysteroscopic endometrial polyps resection, take polyp tissue as experimental group one, away from the polyp intima tissue experimental group two. Paraffin pathology confirmed benign endometrial polyps. The experimental group and the control group were detected in six peripheral blood hormone, estrogen in the two groups were compared by immunohistochemical detection of polyps and endometrial aromatase expression using Image-Pro Plus 6.0 image analysis software Image analysis, the average optical density as a comparative indicator. Results The levels of serum estrogen in the experimental group and the control group were (16.5 ± 2.11) pg / ml and (15.8 ± 2.28) pg / ml, respectively. There was no significant difference in estrogen levels between the two groups The average optical density of aromatase expression in the second and the control groups were (0.101 1 ± 0.001 3), (0.025 5 ± 0.001 0) and (0.024 7 ± 0.000 7), respectively. The experimental group One of the aromatase expression was significantly higher than the experimental group two, the difference was statistically significant (P <0.05); experimental group two aromatase expression and the control group difference was not statistically significant (P> 0.05). Conclusion The formation of endometrial polyps in menopause does not depend on the increase of circulating estrogen levels, which may be related to the overexpression of aromatase in the endometrium.
