为探讨高血压患者出院后社区系统管理对血压及并发症的影响效果,本研究选取2003年2月~2014年1月我院收治的288例高血压患者作为研究对象,采用随机数表法将患者分为观察组和对照组,出院后分别进行社区系统管理和常规随访,比较管理1年后两组患者血压及并发症发生情况。两组患者管理前血压水平比较差异无统计学意义(p>0.05);管理后6月、12月观察组患者收缩压、舒张压均低于对照组((138.3±10.1)/(82.1±6.4)mm Hg比(141.2±10.8)/(88.9±6.8)mm Hg,(128.2±9.1)/(76.4±5.1)mm Hg比(136.4±9.9)/(85.5±5.9)mm Hg)。观察组脑卒中、肾功能不全、眼底病变及冠心病发生率均低于对照组(0.69%比4.86%,2.08%比6.94%,2.08%比9.03%,3.47%比15.28%,p<0.05),且管理后观察组患者的SF-36量表中躯体角色功能、情绪角色功能、躯体健康、躯体疼痛、社会功能、心理健康、活力评分均显著高于对照组(p<0.05)。结果表明,高血压患者出院后社区系统管理建立全方位血压管理方案,能够有效控制血压,降低并发症发生率,可以在临床上进一步推广和使用。
To investigate the effect of community system management on blood pressure and complications after discharge from hypertensive patients, 288 hypertensive patients admitted to our hospital from February 2003 to January 2014 were selected as the research object. The incidence of stroke, renal insufficiency, fundus lesions and coronary heart disease were lower in the observation group than in the control group (0.69% vs. 4.86%, 2.08% vs. 6.94%, 2.08% vs. 9.03%, 3.47% vs 15.28%, p <0.05) , And the body function, emotional function, physical health, somatic pain, social function, mental health and vitality score of SF-36 in the observation group were significantly higher than those in the control group (p <0.05). The results showed that community-based management of hypertensive patients discharged from hospital to establish a comprehensive program of blood pressure management can effectively control blood pressure and reduce the incidence of complications, which can be further promoted and used clinically.
