目的了解宁波地区已婚妇女宫颈病变及人乳头状瘤病毒感染状况及其分布特点。方法采用多阶段整群随机抽样的方法,在宁波地区抽取城镇与乡村地区>20岁已婚妇女8786例行妇科检查、宫颈液基细胞学检查、HC_2 HPV DNA检测及阴道镜下宫颈活检病理检查,进行统计学分析。结果(1)宁波地区宫颈细胞异常率为0.888%,ASC-US 0.546%、LSIL 0.205%、HSIL-0.137%,城乡妇女宫颈细胞异常检出率比较,无统计学差异(P>0.05)。(2)阴道镜下宫颈活检病理结果表明ASC-US病例CIN患病率为16.7%,LSIL符合率66.7%,HISL符合率为83.3%,有统计学差异(P<0.01)。(3)宁波地区妇女HR-HPV感染率为13.3%,城乡妇女比较,无统计学差异(P>0.05)。(4)不同年龄组HR-HPV感染率有统计学差异(P<0.01)。35~44岁HR-HPV感染率明显升高,约16.5%。(5)TCT结果 ASC-US、LSIL、HSIL,HR HPV感染率为20.8%、66.7%、75.0%,有统计学差异(P<0.01)。(6)宫颈上皮内瘤变程度越高,HR-HPV感染率越高(P<0.01)。结论宁波地区宫颈细胞异常率为0.888%,HR-HPV感染率为13.3%;HR-HPV感染的高峰年龄为35~44岁;宫颈液基细胞学检查、阴道镜下宫颈活检病理检查结合HC_2 HPV DNA检测,对宫颈病变的诊断治疗及随访及宫颈癌的预防有重要意义。
Objective To understand the cervical lesions and human papilloma virus (HPV) infection in married women in Ningbo and their distribution characteristics. Methods A total of 8786 gynecological examinations, cervical liquid-based cytology tests, HC2 HPV DNA tests and colposcopic biopsy were performed in urban and rural areas in Ningbo using a multistage cluster randomized sampling method. , For statistical analysis. Results (1) The abnormal rates of cervical cells in Ningbo were 0.888%, ASC-US 0.546%, LSIL 0.205% and HSIL-0.137% respectively. There was no significant difference in the detection rate of cervical cell abnormalities between urban and rural women (P> 0.05). (2) Colposcopic biopsy results showed that the prevalence of CIN in ASC-US cases was 16.7%, LSIL coincidence rate was 66.7%, and HISL coincidence rate was 83.3% (P <0.01). (3) The HR-HPV infection rate of women in Ningbo was 13.3%. There was no significant difference between urban and rural women (P> 0.05). (4) The HR-HPV infection rates in different age groups were statistically different (P <0.01). The prevalence of HR-HPV in 35-44 years old was significantly higher, about 16.5%. (5) The HPV infection rates of ASC-US, LSIL, HSIL and HR were 20.8%, 66.7% and 75.0% respectively by TCT (P <0.01). (6) The higher the degree of cervical intraepithelial neoplasia, the higher the HR-HPV infection rate (P <0.01). Conclusion The abnormal rate of cervical cells in Ningbo is 0.888% and the rate of HR-HPV infection is 13.3%. The peak age of HR-HPV infection is 35-44 years. Cervical liquid-based cytology and colposcopic biopsy combined with HC_2 HPV DNA testing, diagnosis and treatment of cervical lesions and follow-up and prevention of cervical cancer is important.
