目的总结胸锁乳突肌肌瓣在食管癌术后颈部吻合口狭窄中的的应用经验。方法对2005年10月-2010年1月收治的4例食管癌术后颈部吻合口严重狭窄的患者,切开吻合口,根据狭窄部位的周径及长度设计胸锁乳突肌肌皮瓣,予以可吸收线无张力缝合。结果 4例手术全部成功,肌皮瓣无缺血坏死,术后2周患者均能进食,钡剂造影显示无狭窄、梗阻,随访6～18个月疗效满意。结论胸锁乳突肌肌皮瓣在食管癌术后颈部吻合口狭窄的疗效确切,是一种良好的选择。
Objective To summarize the experience of application of sternocleidomastoid muscle flap in anastomotic stenosis after esophagectomy. Methods Four patients with severe stenosis of neck anastomosed esophageal cancer treated from October 2005 to January 2010 were anastomosed and the sternocleidomastoid muscle flap was designed according to the circumference and length of the stenosis , To absorb the tension-free suture line. Results All the 4 surgeries were successful. Myocutaneous flaps were not ischemic and necrosis. All patients were able to eat 2 weeks after operation. Barium contrast showed no stenosis and obstruction. The follow - up of 6 to 18 months was satisfactory. Conclusion Sternocleidomastoid myocutaneous flap is an effective method for the treatment of cervical anastomotic stenosis after esophagectomy.
