目的 :探讨类风湿关节炎 (RA)患者膝关节 B超检查的临床意义。方法 :对正常人对照组膝关节 (n=40个 )及RA组膝关节 (n=94个 )采用 B超进行检测 ,观察膝关节髌上囊积液、滑膜厚度、股骨软骨厚度及软骨下破坏程度。结果 :(1)与对照组比较 ,RA组髌上囊积液明显增多 ,滑膜厚度、股骨软骨厚度明显增厚。 (2 ) RA组功能状况 、 、 、 级患者髌上表滑膜厚度较对照组明显增厚 ,各级患者滑膜厚度有显著差异性 ,随着 RA患者功能状况越严重 ,滑膜厚度逐渐增加。RA组功能状况 、 、 、 级各级患者内髁软骨厚度无差异显著性。RA组 、 、 、 级各级患者膝关节软骨下骨质破坏程度构成比不同 ,随着功能状况越严重 ,骨质破坏程度重度、轻度病例所占比例越多。 (3 )与无膝关节肿痛病史 RA患者比较 ,有膝关节肿痛病史 RA患者膝关节髌上囊积液明显增多 ,滑膜厚度、内髁软骨厚度明显增厚 ,软骨节下骨质破坏严重病例构成比增多。结论 :(1) RA患者膝关节 B超检查作为一种无创性检查在临床诊断及判断功能状况上有其独到的优越性 ,髌上囊滑膜厚度检测是反应 RA患者功能状况较好的指标。 (2 )有膝关节肿痛病史是 RA患者膝关节受损害的重要标志。
Objective: To investigate the clinical significance of B-ultrasound in knee joint of patients with rheumatoid arthritis (RA). Methods: The knee joint (n = 40) and RA group (n = 94) of the normal control group were examined by B ultrasound. RA group functional status, grade, level at all levels of condylar cartilage thickness was no significant difference. The compositional ratio of knee under the subchondral bone in patients with RA at, among, and levels were different. As the functional status became more serious, the degree of bone destruction was severe, and the proportion of mild cases was more. (3) Compared with RA patients without history of knee joint pain, patients with knee joint pain history showed a significant increase of knee joint patellar bursa fluid, thickening of synovial membrane, thickening of condylar cartilage, Serious cases constitute an increase. Conclusions: (1) As a noninvasive examination of knee joint B-ultrasound in RA patients, it has its unique superiority in clinical diagnosis and judgment of functional status. The measurement of synovial thickness of the suprapatellar plexus is a good indicator of the functional status of RA patients . (2) history of knee swelling and pain is an important indicator of knee injury in RA patients.
