目的分析血尿酸、尿微量白蛋白水平与2型糖尿病大血管病变的关系。方法选取2007年3月至2010年3月收治的2型糖尿病患者88例,依据患者的血尿酸水平分为血尿酸升高组(Ⅰ组)49例与血尿酸正常组(Ⅱ组)39例;依据患者尿微量白蛋白的排泄率(UAER)分成两组:其中UAER在200μg/min以上者为Ⅲ组,共52例;UAER在20~200μg/min者为Ⅳ组,共36例。记录各组患者病程、血尿酸(UA)、颈动脉内膜-中层厚度(IMT)以及UAER水平,比较各项目差异,分析血尿酸、尿微量白蛋白水平及糖尿病患者颈动脉内膜病变的关系。结果通过多元回归分析,表明UA、UAER与颈动脉内膜病变呈显著性的正相关,UA与UAER为显著性相关。结论 2型糖尿病患者颈动脉内膜病变受到UA、UREA水平的密切影响,因而可以应用患者UA与UREA水平对2型糖尿病颈动脉内膜病变风险进行早期的预测。
Objective To analyze the relationship between serum uric acid, urine microalbumin and type 2 diabetic macroangiopathy. Methods From March 2007 to March 2010, 88 patients with type 2 diabetes mellitus were enrolled. According to the level of serum uric acid in patients, 49 patients with elevated serum uric acid (group Ⅰ) and 39 patients with normal serum uric acid (group Ⅱ) ; According to the urinary albumin excretion rate (UAER), the patients were divided into two groups: UAER of 200μg / min or more for the group Ⅲ, a total of 52 cases; UAER 20 ~ 200μg / min were Ⅳ group, a total of 36 cases. The duration of disease, serum uric acid (UA), carotid artery intima-media thickness (IMT) and UAER were recorded in each group. The differences of each item were compared. The relationship between serum uric acid and urinary albumin and carotid artery endothelium in diabetic patients was analyzed . The results of multiple regression analysis showed that UA, UAER and carotid artery intimal lesions was significantly correlated positively, UA and UAER was significantly related. Conclusions Carotid artery endometrial lesions in type 2 diabetic patients are closely influenced by UA and UREA levels. Therefore, UA and UREA levels in patients with type 2 diabetes can be used to predict the risk of carotid endarteropathy in type 2 diabetic patients in the early stage.
