目的 :评价因听骨链固定、坏死或脱位而行听骨成形术的临床疗效。方法 :将残存的槌砧骨雕磨成一定形状的骨柱 ,形如倒置的洒杯 ,一端磨出髋臼关节凹 ,另一端磨出槽状关节面 ,镶嵌于镫骨头和槌骨柄之间。槌骨坏死则将砧骨长突改建成“槌骨柄”。镫上结构缺损 ,则将槌砧骨改形后连接于镫骨足板和鼓膜之间。结果 :术后 3月 ,1 71 / 1 96耳听力提高 >1 0 d B,总有效率是87.2 % ;1 4 3 / 1 96耳达到应用水平。骨气—导间距 <2 0 d B占 68% ,1年后为64 %。随访 3~ 5年仍保持稳定的听力水平。结论 :采用自体残存听骨行听骨成形术治疗慢性化脓性中耳炎、先天性中耳畸形及外伤性听骨链断裂是恢复听力的有效中耳传音机制重建的方法
OBJECTIVE: To evaluate the clinical efficacy of metacarpal ossification for the fixation of ossicular chain, necrosis or dislocation. Bone - guide spacing <20 d B accounted for 68%, 64% after 1 year. Follow-up 3-5 years still maintain a steady level of hearing. CONCLUSIONS: Autologous residual ossicular osteotomy for the treatment of chronic suppurative otitis media, congenital middle ear deformity, and traumatic ossicular chain rupture are effective methods of restoring auditory middle ear acoustic mechanisms
