目的评估Kieland产钳对持续性枕后位进行反置上钳旋转胎头的安全性及可行性。方法对100例胎头双顶径已达或已过坐骨棘水平的持续性枕后位产妇,用Kieland产钳进行反置上钳旋转胎头产钳术。结果成功率100%,母体软产道损伤17%,新生儿损伤12%,无会阴Ⅲ度裂伤,无后穹窿、宫颈、膀胱损伤及子宫破裂,无新生儿颅内出血及死亡等严重并发症。结论Kieland产钳反置上钳旋转胎头术用于双顶径已达或已过坐骨棘水平的持续性枕后位是安全、方便、可行的,并对降低剖宫产率具有一定的意义。
Objective To assess the safety and feasibility of Kieland forceps for reversing the posterior position of the fetal head with rotating forceps. Methods A total of 100 cases of persistent occipitocervical mothers who had a bipolar vertebral bivalve at the level of orbicular spine had a Kieland forceps for revolving captop forceps. Results The success rate was 100%, maternal soft birth injury was 17%, neonatal injury was 12%, no perineal Ⅲ degree laceration, no posterior fornix, cervix, bladder injury and uterine rupture, no serious complications such as neonatal intracranial hemorrhage and death. Conclusion It is safe, convenient and feasible to use Kieland forceps reverse forceps rotation fetal capitoplasty for persistent posterior position of bicuspids with or without isoptinal spine. It is of great significance to reduce the rate of cesarean section.
