目的探讨燃煤型砷中毒病人的微循环变化及其临床意义。方法应用LWB—I型微循环仪对162例砷中毒病人及68例正常人作双眼球结膜微循环观测。结果不同程度砷中毒病人,其球结膜微循环变化也不尽相同。轻度砷中毒病人以球结膜微血管数目减少及血管周围渗出为主;中度砷中毒病人以血管粗细不均,血流速度缓慢较为显著;重度砷中毒病人的各项指标的异常发生率逐渐升高,尤以血球重度聚集及缺血区的出现更为突出。结论燃煤型砷中毒病人的重要病理改变是球结膜微循环障碍。燃煤中的砷对人体造成损伤,可直接和间接的损害微血管,可使微血管管腔闭塞,管壁退变吸收,形成不可逆性缺血区,从而引起微循环的障碍。砷中毒病人的球结膜微循环障碍与其病情呈正相关。球结膜微循环障碍是慢性砷中毒病 人的重要表现,监测其变化,有助于砷中毒病因的探讨、病情的判断及指导治疗。
Objective To investigate the changes of microcirculation in patients with coal-burning arsenism and its clinical significance. Methods 162 cases of arsenism and 68 normal subjects were examined by LWB-I microcirculation. Results varying degrees of arsenic poisoning, changes in the bulbar conjunctiva microcirculation are not the same. Patients with mild arsenism had fewer conjunctival capillaries and perivascular exudation; patients with moderate arsenic poisoning had uneven blood vessel thickness and slow blood flow; patients with severe arsenism had a gradual rate of abnormalities in various indicators Rise, especially in severe blood cell aggregation and the emergence of ischemic area is more prominent. Conclusion An important pathological change in patients with coal-burning arsenism is bulbar conjunctival microcirculation. Coal in the arsenic cause damage to the human body, can directly and indirectly damage the microvascular, can microvascular lumen occlusion, wall degeneration and absorption, the formation of irreversible ischemic area, causing microcirculation obstacles. Arsenic poisoning conjunctival microcirculation disorder and its disease was positively correlated. Bulbar conjunctival microcirculation is an important manifestation of chronic arsenic poisoning patients to monitor its changes, contribute to the etiology of arsenic poisoning to determine the condition and guide the treatment.
