目的观察结直肠腺癌肠周淋巴结检出数量、淋巴结转移数量与PTEN的关系,探讨其临床价值。方法分析101例结直肠腺癌患者检出的肠周淋巴结数量及淋巴结转移情况与术后组织中PTEN的表达相关性。结果 101例患者检出淋巴结数量为1~72枚,平均17枚。伴有淋巴转移者47例占46.53%,转移淋巴结数量为1~15个。淋巴结检出数量≤12个者35例,其中PTEN的阳性率为14%(5/35);淋巴结检出数量>12个者66例,其中PTEN的阳性率为33%(22/66)。有淋巴结转移患者PTEN的阳性率为15%(7/47),无淋巴结转移患者PTEN的阳性率为37%(20/54),经线性相关分析显示结直肠腺癌患者淋巴结检出数量与PTEN的表达具有正相关性(r=0.46,P=0.039 2),转移淋巴结数量与PTEN的表达具有负相关性(r=-0.49,P=0.041 0)。结论结直肠腺癌中检出淋巴结数量和转移的淋巴结数量均与PTEN的表达具有相关性,PTEN作为肿瘤转移抑制基因可能对肿瘤发生和进展时肠周淋巴结的反应性改变和淋巴结转移有一定影响。结直肠腺癌术后患者积极关注肠周淋巴结检出数量和PTEN的表达,可能对判断生物学行为有重要意义。
Objective To observe the number of colorectal lymph nodes and the relationship between lymph node metastasis and PTEN in colorectal adenocarcinoma and to explore its clinical value. Methods The correlation between the number of lymph nodes and the lymph node metastasis in 101 cases of colorectal adenocarcinoma and the expression of PTEN in the tissues were analyzed. Results The number of lymph nodes in 101 patients was from 1 to 72, with an average of 17. 47 cases with lymph node metastasis accounted for 46.53%, the number of metastatic lymph nodes was 1 ~ 15. The positive rate of PTEN was 14% (5/35). The number of lymph nodes was> 12 in 66 cases. The positive rate of PTEN was 33% (22/66). The positive rate of PTEN in patients with lymph node metastasis was 15% (7/47), and the positive rate of PTEN in patients without lymph node metastasis was 37% (20/54). The linear correlation analysis showed that the number of PTEN in patients with colorectal adenocarcinoma was significantly higher than PTEN (R = 0.46, P = 0.039 2). There was a negative correlation between the number of metastatic lymph nodes and the expression of PTEN (r = -0.49, P = 0.041 0). Conclusions The number of lymph nodes and the number of metastatic lymph nodes in colorectal adenocarcinoma are correlated with the expression of PTEN. PTEN, as a tumor metastasis suppressor gene, may have some influence on the reactivity change and lymph node metastasis of peritumoral lymph nodes in tumorigenesis and progression . Patients with colorectal adenocarcinoma after active focus on the number of intestinal lymph nodes and the expression of PTEN may be important in judging the biological behavior.
