目的了解福辛普利对老年高血压病患者血管内皮依赖性舒张功能(FMD)和血管内皮内分泌功能的影响。方法应用高频彩色多普勒超声检测仪对82例老年高血压病患者进行FMD的无创检查了解血管内皮依赖性舒张功能,同时测定血浆内皮素1(ET-1)水平了解血管内皮内分泌功能,观察福辛普利治疗6个月前后患者FMD及ET-1水平的变化。结果福辛普利治疗6个月后老年高血压病患者肱动脉基础血管内径和硝酸甘油诱发的肱动脉内径变化率与治疗前差异无统计学意义(P>0.05),但反应性充血诱发的肱动脉内径变化率较治疗前明显增加(P<0.05),同时ET-1水平明显下降(P<0.05)。结论老年高血压病患者存在明显FMD受损和血管内皮内分泌功能障碍,福辛普利治疗可改善老年高血压病患者FMD,降低ET-1水平,改善血管内皮功能。
Objective To investigate the effect of fosinopril on endothelium-dependent vasodilation (FMD) and endocrine function in elderly patients with essential hypertension. Methods 82 cases of elderly hypertensive patients were examined by non-invasive FMD with high-frequency color Doppler ultrasonography to understand the endothelium-dependent vasodilatation and the level of endothelin-1 (ET-1) To observe the changes of FMD and ET-1 levels in patients treated with fosinopril for 6 months. Results The changes of basal vascular diameter of brachial artery and brachial artery diameter induced by nitroglycerin in elderly hypertensive patients after 6 months of treatment with fosinopril were not statistically different from those before treatment (P> 0.05), but the changes of brachial artery induced by reactive hyperemia The change rate of brachial artery diameter was significantly higher than that before treatment (P <0.05), meanwhile ET-1 level was significantly decreased (P <0.05). Conclusions There is obvious impairment of FMD and endocrine dysfunction in elderly hypertensive patients. Fosinopril can improve FMD, decrease ET-1 and improve endothelial function in elderly hypertensive patients.
