目的探讨树突状细胞(dendritic cells,DC)调控信号的异常与过敏性紫癜(Henoch-Schonlein purpura,HSP)发病中T辅助细胞分化失衡的关系。方法分别采用散射比浊法、ELISA和流式细胞技术,检测50例HSP患儿和20例健康儿童血浆中免疫球蛋白、血浆IL-4、INF-γ和DC培养上清IL-12、IL-10水平及DC表面CD86、CD80、CD83、HLA-DR、CD-209的表达情况。结果与对照组(健康儿童,n=20)比较,HSP组患儿:血浆免疫球蛋白IgA及IgE水平升高;血浆中Th2类细胞因子IL-4升高,Th1类细胞因子INF-γ降低;DC培养上清中IL-10升高,IL-12降低;DC表达CD86、CD83、HLA-DR水平升高,表达CD80水平降低(P<0.05)。CD-209在两组之间差异无统计学意义(P>0.05)。结论DC的抗原特异性信号、协同刺激信号和分化或极化信号3种调控信号的异常,与HSP患儿Th1/Th2失衡、免疫球蛋白合成异常密切相关。
Objective To investigate the relationship between dendritic cells (DCs) regulatory signal abnormalities and imbalance of T helper cells in the pathogenesis of Henoch-Schonlein purpura (HSP). Methods The plasma immunoglobulin, plasma IL-4 and INF-γ in 50 children with HSP and 20 healthy children were detected by nephelometry, ELISA and flow cytometry respectively. The supernatant of IL-12, IL -10 levels and the expression of CD86, CD80, CD83, HLA-DR and CD-209 on the DC surface. Results Compared with the control group (n = 20), the levels of plasma IgA and IgE increased in plasma of HSP group; the levels of Th2 cytokines IL-4 and INF-γ of Th1 cytokines decreased IL-10 was increased and IL-12 was decreased in DC culture supernatant. CD86, CD83 and HLA-DR were increased in DCs and CD80 was decreased in DCs (P <0.05). There was no significant difference in CD-209 between the two groups (P> 0.05). Conclusion The abnormal signals of DCs, such as antigen-specific signals, costimulatory signals and differentiated or polarized signals, are closely related to the imbalance of Th1 / Th2 and immunoglobulin synthesis in children with HSP.
