Harmonic knife is a new surgical device that uses ultrasonic energy to coagulate-cut tissue. Here, we introduce to you the preliminary experience of separating the pancreatic parenchyma and preventing the formation of pancreatic fistula with a harmonic knife in pancreatic tail resection. METHODS: Between April 1998 and July 1999, we performed 13 pancreatic tail resections with a harmonic knife. The average age of these patients was 64 years old. One patient had a history of drinking alcohol. One patient had gastric resection of gastric cancer three years ago. Of the 13 patients, 11 had gastric cancer, one of whom was gastric stump cancer and had a combined pancreas and pancreas tail.
