目的探讨糖尿病患者早期肾损伤指标。方法 217例2型糖尿病患者分为胱抑素C(Cystatin C,CYS-C)正常组62例和CYS-C异常组155例。测定血、尿常规、血清CYS-C、尿微量白蛋白(MAU)、糖化血红蛋白(HbA1c)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血尿酸(UA)等血液生化指标并分析其与CYS-C的相关性。结果 CYS-C异常组年龄、肾小球滤过率、血尿酸明显高于CYS-C正常组(P<0.05);CYS-C与MAU、肌酐、尿素氮、血尿酸水平呈正相关,而与肾小球滤过率呈负相关。结论血CYS-C和MAU是肾脏早期损伤的重要标志,联合检测简便、灵敏,对及时准确地诊断糖尿病早期肾损害有非常重要的临床应用价值,值得在临床上推广应用。
Objective To investigate the indicators of early renal injury in diabetic patients. Methods Totally 217 patients with type 2 diabetes were divided into two groups: control group (CYS-C), normal group (62 cases) and CYS-C abnormal group (155 cases). Blood, urine routine, serum CYS-C, MAU, HbA1c, TG, TC, HDL-C, (LDL-C), serum uric acid (UA) and other blood biochemical parameters and analyze its correlation with CYS-C. Results The age, glomerular filtration rate and serum uric acid in CYS-C abnormal group were significantly higher than those in CYS-C normal group (P <0.05). CYS-C was positively correlated with MAU, creatinine, urea nitrogen and serum uric acid, Glomerular filtration rate was negatively correlated. Conclusions Blood CYS-C and MAU are important markers of early renal injury. The combined detection is simple and sensitive. It has very important clinical value in diagnosing early diabetic nephropathy. It is worth to be popularized clinically.
