目的观察可控鼓泡式正压呼吸支持装置对需要呼吸支持患儿的临床治疗效果。方法对我院PICU、NICU及C ICU中行无创呼吸支持的113例患儿进行临床疗效观察。将所有患儿随机分为两组,其中治疗组(53例)在常规治疗基础上给予可控鼓泡式正压呼吸支持装置-鼻塞式气道持续正压通气(NCPAP)呼吸支持,对照组(60例)在常规治疗基础上给予德国STEPHAN CPAP-A呼吸机NCPAP呼吸支持,动态监测两组患儿治疗前、治疗后1、2、8、12 h的动脉血气,并对两组患儿插管机械通气发生率进行比较。结果治疗后两组患儿血气指标比较差异无统计学意义(P>0.05),插管机械通气发生率比较差异无统计学意义(P>0.05)。结论可控鼓泡式正压呼吸支持装置对需要初步呼吸支持的患儿临床治疗效果良好,能达到同类机器的治疗效果,且结构简单,操作方便,价格低廉,适合在基层医院中推广使用。
Objective To observe the clinical effect of controlled bubbling positive pressure respiratory support device on children with respiratory support. Methods The clinical efficacy of 113 children with noninvasive respiratory support in PICU, NICU and C ICU in our hospital was observed. All patients were randomly divided into two groups. The treatment group (n = 53) received controlled bubbling positive pressure respiratory support device (NCPAP) respiratory support on the basis of routine treatment. The control group (60 cases) were given the respiratory support of STEPHAN CPAP-A ventilator NCPAP on the basis of routine treatment. The arterial blood gas of two groups before and after treatment was monitored at 1, 2, 8 and 12 h after treatment. Intubation mechanical ventilation rates were compared. Results There was no significant difference in blood gas index between the two groups after treatment (P> 0.05). There was no significant difference in the incidence of intubation mechanical ventilation (P> 0.05). Conclusions Controllable bubbling positive pressure respiratory support device is effective in treating children who need primary respiratory support and can achieve the same therapeutic effect. The device is simple in structure, convenient in operation and low in price, and is suitable for promotion and use in primary hospitals.
