目的探讨眼底荧光血管造影与光学相干断层扫描在原田病诊断中的意义。方法对8例16眼原田病患者行FFA及OCT检查,全部病例均应用糖皮质激素冲击治疗,1周后复查OCT。结果 8例16眼原田病患者FFA检查表现为双眼早期视网膜色素上皮多发性渗漏,晚期多湖状荧光素积存的特征性外观,OCT显示多发的连续的神经上皮浆液性脱离,部分液性暗区内可见稀疏的蓝色反光点,经糖皮质激素冲击治疗1周后,OCT复查浆液性渗出迅速吸收。结论在原田病早期以双眼后极部多发的浆液性视网膜脱离为主,联合FFA与OCT可以对其早期作出正确诊断,同时为疾病的病情追踪、疗效评价以及视力预测提供了客观定量的评价标准。
Objective To investigate the significance of fundus fluorescein angiography and optical coherence tomography in the diagnosis of Harada disease. Methods FFA and OCT were performed in 8 cases of Harada patients in 8 cases. All cases were treated with glucocorticoid shock therapy and OCT was performed one week later. Results The FFA of 8 cases of Harada patients showed the characteristic appearance of multiple leakage of early retinal pigment epithelium in both eyes and late lakes fluorescein accumulation. OCT showed multiple continuous neuroepithelial serous detachment and partial liquid darkness Visible sparse blue reflective spots in the area, the impact of glucocorticoid treatment for 1 week, OCT reabsorption rapid serous excretion. Conclusions In the early stage of Harada disease, the majority of serous retinal detachment occurs mainly in the posterior pole of both eyes. Combined with FFA and OCT, it can be used to diagnose the disease early and provide objective and quantitative evaluation criteria for disease tracking, curative effect evaluation and vision prediction .
