胰岛素泵(insulin pump,IP)在短期内使血糖迅速达标并改善胰岛β细胞功能的报道陆续发表后[1-2],引起了人们极大的兴趣,并进行了大量的研究,同其他降糖方案相比,无论是疗效还是安全性上都更具优势[3-10],如今已经广泛应用于临床的治疗。胰岛素泵除了可以设定24 h分段基础率以外,它还有一些不同的餐前追加量输注模式,例如单波(Single-wave bolus,SW)输注和双波(dual-wave bolus,DW)输注。SW即餐前一次性给予皮下胰岛素追加量;DW则是将总量分解为两部分,即一部分以SW的形式给予,紧接着再给予“方波”(给定胰岛素在设定时间段内平均给予),形式上,DW相当于“SW”联合“方波”。在不同饮食条件下,患者血糖波动的情况可能差异很大。处于新疆的患者,少数民族和汉族的饮食习惯相互影响,形成了目前高碳水化合物、高脂、高蛋白的地方饮食特点,血糖常常呈现“高而宽”的波动。探讨SW和DW输注模式,评价其在这一群体中的疗效,安全性及使用特点,有利于胰岛素泵更
