探讨彩色多普勒能量图 (CDE)在小肝癌鉴别诊断中的应用价值。方法 利用彩色多普勒能量图并结合彩色多普勒血流成像 (CDFI)对 37例共 5 3个病灶的肝脏实质性占位病变的患者进行观察 ,观察其病灶周边及内部血流灌注情况。结果 肝脏恶性肿瘤病灶周边的内部血流信号丰富呈“树枝状”及“星形”多为搏动性血流 ,且原发性肿瘤显示率 >继发性肿瘤 ;血管瘤血流呈“火球状”。未见有搏动性血流显示 ;非肿瘤占位性病变病灶内未见血流显示。结论 彩色多普勒能量图 (CDE)具有非角度依赖性及显示信号范围广泛等特点 ,能较完整地显示微小及弯曲迂回的血管并能有效地显示低速血流或平均速度为零的灌注区 ,而恶性肿瘤与血管瘤及非肿瘤性占位的血供方面存在差异 ,这使 CDE在肝脏小占位病变的鉴别诊断方面的应用具有较大价值。
To explore the value of color Doppler energy imaging (CDE) in the differential diagnosis of small liver cancer. Methods Color Doppler energy maps and color Doppler flow imaging (CDFI) were used to observe 37 cases of liver lesions with a total of 53 lesions and observe the peripheral and internal blood flow . Results The internal blood flow signals around the lesions of hepatic malignancies were “dendritic” and “star” mostly pulsatile blood flow, and the rate of primary tumor was> secondary tumor; the blood flow of hemangioma was “fireball ”. There was no pulsatile blood flow showed; non-tumor lesions lesions showed no blood flow. Conclusion The color Doppler energy map (CDE) is non-angular dependent and displays a wide range of signals. It can display small and tortuous blood vessels and can effectively display low blood flow or perfusion area with average velocity of zero , While there is a difference in the blood supply between malignant tumors and hemangiomas and non-neoplastic tumors, which makes the application of CDE in the differential diagnosis of small liver lesions of great significance.
