目的:研究通过前哨淋巴通道(sentinel lymph channel,SLC)行前哨淋巴结活检(sentinel lymph node biopsy,SLNB)指导乳腺癌腋窝淋巴结分期的可行性。方法:采用99 m锝硫胶体和亚甲蓝联合示踪法,通过SLC行SLNB,对检取的前哨淋巴结(sentinel lymph node,SLN)行连续切片和苏木精-伊红染色法(HE)检查。结果:2008年7月~2010年8月30例早期乳腺癌患者成功行SLNB,检出率为96.8%(30/31),假阴性率为0(0/13)。检出SLN 1~3枚,共39枚,平均1.3枚。连续切片和HE染色检出阳性SLN 13例,其中微转移2例,阴性SLN 17例。结论:通过SLC行SLNB准确可靠,连续切片技术是准确评估SLN的重要病理检测方法,有助于精确指导乳腺癌的腋窝淋巴结分期。
Objective: To investigate the feasibility of sentinel lymph node biopsy (SLNB) to guide the axillary lymph node staging of breast cancer through the sentinel lymph channel (SLC). Methods: Sentinel lymph node (SLN) was serially sectioned and hematoxylin-eosin (HE) staining was performed by SLC line SLNB with 99mTc-Sulfur colloid and methylene blue combined with tracer method. an examination. Results: From July 2008 to August 2010, 30 cases of early breast cancer were successfully treated with SLNB. The detection rate was 96.8% (30/31) and the false negative rate was 0 (0/13). SLN detected 1 to 3, a total of 39, an average of 1.3. Thirteen cases of positive SLN were detected by serial sections and HE staining, including 2 cases of micrometastasis and 17 cases of negative SLN. CONCLUSIONS: The SLNB is accurate and reliable through SLC. The serial slice technique is an important pathological method to accurately evaluate SLN, which helps to accurately guide the axillary lymph node staging of breast cancer.
