目的探讨18F标记的脱氧葡萄糖正电子发射计算机体层成像(18 F-FDG PET/CT)全身显像在诊断恶性黑色素瘤术后局部复发、转移与预后中的临床价值。方法回顾性分析原发灶均明确病理诊断的恶性黑色素瘤术后患者30例,共行18 F-FDG PET/CT全身显像36人次,测量肿瘤及转移部位最大标准摄取值(SUVmax)。结果随访至确诊后10-72个月。18 F-FDG PET/CT全身显像发现转移病灶并再分期患者19例,其中11例分期升高,共发现转移58处,包括淋巴结26处、软组织内14处、骨骼灶11处,SUVmax值分别为7.92±3.84、5.80±4.28和8.16±4.37;其余为脑、胰腺各2处,肺、肝、甲状腺转移各1处。另外6例Ⅱ期患者复发但无转移。18F-FDG PET/CT显像阴性的5例患者随访结束时均健在。结论 18F-FDG PET/CT全身显像对恶性黑色素瘤再分期与预后有较高的临床应用价值。
Objective To investigate the clinical value of 18F labeled 18F-FDG PET / CT whole body scintigraphy in the diagnosis of local recurrence, metastasis and prognosis of malignant melanoma. Methods Thirty patients with malignant melanoma with definite pathological diagnosis of primary tumor were retrospectively analyzed. Total 18 F-FDG PET / CT scans were performed 36 times. The maximal standard uptake (SUVmax) of tumor and metastasis was measured. Results were followed up to 10-72 months after diagnosis. Twenty-nine patients with metastatic disease and re-staging were found in 18 F-FDG PET / CT. Among them, 11 cases were elevated in stages, including 58 metastases, including 26 lymph nodes, 14 soft tissues, 11 skeletal lesions and SUVmax values Respectively, 7.92 ± 3.84,5.80 ± 4.28 and 8.16 ± 4.37; the rest of the brain, the pancreas of the two, lung, liver and thyroid metastasis of a. Another 6 patients with stage Ⅱ recurrence but no metastasis. Five patients with negative 18F-FDG PET / CT imaging were alive at the end of follow-up. Conclusion 18F-FDG PET / CT whole body imaging has a high clinical value for malignant melanoma re-staging and prognosis.
