Pulmonary congestion in patients with left ventricular disease is generally considered associated with decreased ventricular systolic function. Although this is usually the case, the pulmonary rales, gallop, and pulmo- nary venous congestion on chest radiographs do not necessarily all reflect impairment of ventricular contractions and may indicate relaxation compliance or diminished capacity to expand left ventricle. Indeed, many cases of pulmonary congestion may have low ventricular systolic and cardiac output, but there are cases of ventricular systolic function, or systolic ejection ratio higher than normal (such as hypertrophic cardiomyopathy).
