晚期帕金森病(PD)常并发视幻觉、偏执狂样妄想及精神混乱等精神病。长期应用左旋多巴及其它抗PD药物治疗可以导致精神障碍,精神病是PD恶化的一种标志。突触后超敏多巴胺受体的过度刺激及5—羟色胺(5—HT)能神经系统受损可能是造成PD病人精神症状的机制。PD病人的精神症状可能系多巴胺的增加导致5—HT释放增多,使皮层突触后5—HT受体受到过度刺激的结果。基于此,作者采用选择性5—HT_3受体拮抗剂ondansetron治疗16例PD病人的精神症状。
Late Parkinson’s disease (PD) often complicated by visual hallucinations, paranoid delusions and mental disorders such as mental illness. Long-term use of levodopa and other anti-PD drug treatment can lead to mental disorders, mental illness is a sign of PD deterioration. Overexpression of postsynaptic hypersensitive dopamine receptors and impaired serotonergic (5-HT) neuronal system may be the mechanisms responsible for the psychiatric symptoms in patients with PD. The psychiatric symptoms of PD patients may result from an increase in 5-HT release due to an increase in dopamine that results in excessive stimulation of the post-synaptic 5-HT receptors in the cortex. Based on this, the authors used a selective 5-HT_3 receptor antagonist ondansetron treatment of psychiatric symptoms in 16 patients with PD.
