目的探讨内皮型一氧化氮合酶(eNOS)基因第七外显子G894T变异与腰臀比(WHR)异常及其交互作用对原发性高血压的影响。方法对从青岛市4个社区中筛检出的235例原发性高血压患者(病例组)及240例血压正常者(对照组)进行调查,采用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)方法检测G894T变异;应用相加模型分析G894T变异与腰臀比异常的交互作用。结果病例组T等位基因频率(18.94%)及腰臀比异常率(69.79%)与对照组(分别为12.71%、47.92%)比较,差异均有统计学意义(χ2值分别为6.927、23.433,P<0.01)。调整年龄、性别、体质指数、饮酒、吸烟、血脂因素后,G894T基因变异与腰臀比异常的协同效应指数(S)为1.542,归因交互效应〔I(AB)〕为0.909,归因交互效应百分比(AP%)为25.34%,纯因子间归因交互效应百分比(AP*%)为35.14%。结论eNOS基因第七外显子G894T变异与原发性高血压有关,G894T基因变异与腰臀比异常具有正交互作用,控制腰臀比异常可以降低居民原发性高血压患病的危险性。
Objective To investigate the effects of abnormal G894T mutation and WHR on the occurrence of essential hypertension in endothelial nitric oxide synthase (eNOS) gene and its interaction. Methods A total of 235 essential hypertension patients (case group) and 240 normotensive subjects (control group) were screened from 4 communities in Qingdao. The polymerase chain reaction - restriction fragment length polymorphism (PCR-RFLP) was used to detect G894T mutation. An additive model was used to analyze the interaction between G894T mutation and abnormal waist-hip ratio. Results The frequencies of T allele (18.94%) and waist-hip ratio (69.79%) were significantly different from those of the control group (12.71%, 47.92% respectively) (χ2 = 6.927,23.433 , P <0.01). After adjusting for age, gender, body mass index, alcohol consumption, smoking, and lipids, the synergistic effect index (S) of G894T gene mutation and waist-hip ratio was 1.542, and the attributable interaction effect (I The effect percentage (AP%) was 25.34%, and the percentage of attributional interaction effect (AP *%) among pure factors was 35.14%. Conclusion The mutation of G894T in exon 7 of eNOS gene is associated with essential hypertension. The mutation of G894T gene has an orthogonality interaction with abnormal waist-to-hip ratio. Controlling the abnormal waist-hip ratio may reduce the risk of residents with essential hypertension.
