1例69岁女性患者因反复咳嗽、咳痰4个月、乏力1个月入院。电解质检查示血钾2.4 mmol/L;心电图检查示T波低平,T波与U波融合,按心率校正的QT(QTc)间期468 ms。诊断为低钾血症,给予补钾治疗2 d,血钾为3.2~3.3 mmol/L。追问病史得知患者因咳嗽自行服用复方甘草片已2个月,总量超过500片。停用复方甘草片,继续给予补钾治疗,次日患者血钾即升至4.4 mmol/L,乏力症状缓解,心电图检查示T波及QTc间期恢复。停用复方甘草片3 d后停用补钾药物,患者血钾维持在3.6~3.9 mmol/L。随访3个月,患者血钾未再出现异常。
A 69-year-old female patient was admitted for hospitalization due to repeated cough and sputum for 4 months. Electrolyte examination showed potassium 2.4 mmol / L; electrocardiogram showed low T wave, T wave and U wave fusion, heart rate corrected QT (QTc) interval 468 ms. Diagnosis of hypokalemia, giving potassium treatment 2 d, potassium potassium 3.2 ~ 3.3 mmol / L. Asked the medical history that patients taking their own compound licorice tablets for cough for 2 months, the total more than 500 tablets. The compound licorice tablets were discontinued and potassium supplementation was continued. The patient’s potassium level increased to 4.4 mmol / L on the next day, and the symptoms of fatigue were relieved. The T wave and QTc interval recovery showed on ECG. 3 days after the withdrawal of compound licorice tablets disabled potassium drugs, the patient’s serum potassium maintained at 3.6 ~ 3.9 mmol / L. Follow-up 3 months, the patient no longer abnormal serum potassium.
