目的探讨血清C-反应蛋白(CRP)及糖化血红蛋白(Hb A1c)与妊娠期糖尿病(GDM)的相关性,为早期诊断和筛查GDM提供科学依据。方法选取2012年5月-2015年12月于海门市人民医院进行产前检查(产检)的孕妇109例,入选者均于妊娠24~28周进行GDM筛查,均进行75 g口服葡萄糖耐量试验(OGTT)。根据结果将孕妇分为两组:GDM组(66例)及正常糖耐量(NGT)组(43例)。测定并比较两组孕妇血清Hb A1c及CRP水平。结果 GDM组患者血清CRP及Hb A1c水平明显高于NGT组,差异有统计学意义(t=20.11、89.48,P<0.05)。GDM危险因素中孕前体质指数(BMI)、有无糖尿病(DM)家族史及多囊卵巢综合征(PCOS)与血清CRP水平相关(t=16.87、10.12、11.10,P<0.05);孕前BMI、有无DM家族史、不良孕产史及PCOS与Hb A1c水平相关(t=5.83、5.21、2.73、12.68,P<0.05);GDM组患者血清CRP与Hb A1c的秩相关分析显示两者存在正相关(r=0.51,P=0.025)。结论血清CRP和Hb A1c联合检测可作为GDM早期诊断和筛查的有价值指标。
Objective To investigate the correlation between serum C-reactive protein (CRP) and glycosylated hemoglobin (Hb A1c) and gestational diabetes mellitus (GDM), so as to provide a scientific basis for early diagnosis and screening of GDM. Methods A total of 109 pregnant women undergoing prenatal examination at Haimen People’s Hospital from May 2012 to December 2015 were enrolled in this study. All subjects underwent GDM screening 24 to 28 weeks gestation. All patients underwent 75 g oral glucose tolerance test OGTT). According to the results, pregnant women were divided into two groups: GDM group (66 cases) and normal glucose tolerance group (43 cases). The levels of serum Hb A1c and CRP in the two groups were measured and compared. Results The levels of serum CRP and Hb A1c in patients with GDM were significantly higher than those in NGT patients (t = 20.11, 89.48, P <0.05). Pre-pregnancy body mass index (BMI), family history of diabetes mellitus (DM) and polycystic ovary syndrome (PCOS) were correlated with serum CRP levels in GDM risk factors (t = 16.87,10.12,11.10, P < There was no correlation between family history of DM, history of adverse pregnancy and the level of PCOS and Hb A1c (t = 5.83,5.21,2.73,12.68, P <0.05). The rank correlation analysis between serum CRP and Hb A1c in patients with GDM showed that there existed positive Correlation (r = 0.51, P = 0.025). Conclusion Combined detection of serum CRP and Hb A1c can be used as a valuable indicator for the early diagnosis and screening of GDM.
