目的研究不同年龄组乙肝六项测定指标阳性率情况,为慢性乙型肝炎的治疗和预防提供参考。方法对我院2013年5月~2016年5月收治的1872例肝病患者采用酶联免疫吸附法进行乙肝六项检查,对检查结果进行回顾性分析。结果 HBs Ag和HBe Ab检测阳性率较高,分别为81.4%和76.2%,HBe Ag、HBe Ab、HBs Ab、Pre S1Ag分别为19.4%,23.5%、23.3%、29.9%。年龄分布上,31~40岁患者所占比最高达到23.1%,明显高于其他年龄段,年龄在10岁以下的儿童组HBe Ag检出率为28.1%%,高于其他年龄组,差异具有统计学意义(P<0.05)。结论 HBsA g作为HBV的外壳,患者感染HBV后最早可能被验出;而HBe Ab为非保护性抗体,阳性表现与病毒复制和传染能力减弱存在关联,为既往感染标志物。本研究中HBs Ag和HBe Ab检测表现高阳性率说明,肝病患者中,HBs Ag携带者和HBV既往感染占比较高,病毒携带者和既往感染同样存在免疫应答,儿童HBe Ag检出率较高考虑与儿童乙肝多为急性感染,成人病毒携带占比较高有关。可依托乙肝六项检查情况对患者进行乙肝诊断,结合患者临床症状开展治疗工作。
Objective To study the positive rate of six indicators of hepatitis B in different age groups and provide reference for the treatment and prevention of chronic hepatitis B. Methods 1872 cases of liver disease admitted to our hospital from May 2013 to May 2016 were enrolled in this study. Six cases of hepatitis B were detected by enzyme-linked immunosorbent assay, and the results were retrospectively analyzed. Results The positive rates of HBs Ag and HBe Ab were 81.4% and 76.2%, respectively. The HBe Ag, HBe Ab, HBs Ab and Pre S1Ag were 19.4%, 23.5%, 23.3% and 29.9% respectively. The age distribution of patients aged 31 to 40 accounted for the highest proportion of 23.1%, significantly higher than other age groups, children under 10 years of age HBeAg detection rate was 28.1 %%, higher than the other age groups, the difference was Statistical significance (P <0.05). Conclusion As the outer shell of HBV, HBsA g may be detected as early as possible after HBV infection. However, HBeA is non-protective antibody. The positive expression of HBsA g is associated with the reduction of viral replication and infection and is a marker of previous infection. The high positive rate of HBs Ag and HBe Ab in this study indicates that there is a high proportion of HBs Ag carriers and previous HBV infections in patients with liver disease. The carriers of virus and previous infection also have an immune response, and the detection rate of HBe Ag in children is high Consider more than hepatitis B children with acute infection, adult virus carries a higher proportion. Hepatitis B can be diagnosed on the basis of the six checkups of hepatitis B, and the treatment is carried out according to the clinical symptoms of the patient.
