目的探索无偿献血者血液筛检策略,寻找最小试剂成本的最佳筛检方案。方法采用临床经济学最小成本分析方法,从11个筛检试验不同的排列组合所形成的3991.68万个不同的筛检方案,选出15个方案进行分析。结果采血前用HBsAg金标法筛检的方案,试剂成本最高,分别为83318.00元/1000人和82304.98元/1000人。以抗-HIV两种筛检试验均纳入逐项淘汰的方案,试剂成本最低,分别为58674.15元/1000人和58494.70元/1000人。结论应用最小成本分析法组成的筛检方案,可为最小试剂成本的最佳筛检策略提供依据。
Objective To explore blood screening strategies for unpaid blood donors and find the best screening program for the minimum reagent cost. Methods The minimum cost analysis method of clinical economics was used. From the 39 screening and testing trials with different arrangements, 39,916,800 different screening plans were selected and 15 options were selected for analysis. RESULTS: The protocol for screening by HBsAg gold standard before blood sampling had the highest reagent costs, which were 83,318.00 yuan/1000 persons and 82304.98 yuan/1000 persons respectively. Both anti-HIV screening tests were included in the item-by-item phase-out scheme, with the lowest reagent costs, which were 58674.15 yuan/1000 persons and 58494.70 yuan/1000 persons respectively. Conclusion The application of the screening scheme consisting of the minimum cost analysis method can provide the basis for the optimal screening strategy for the minimum reagent cost.
