目的:分析三阶梯止痛治疗失败的重度癌痛患者的疼痛原因及进一步处理方法。方法:收集我科从2012年9月~2015年4月常规三阶梯治疗失败的重度疼痛患者,再次分析疼痛原因,并给予进一步处理,评价疼痛控制效果。结果:共纳入难治性疼痛患者28例,分析其疼痛原因再给予进一步处理。其中疼痛完全缓解为14例,部分缓解为9例,稍有减轻为5例。结论:重度疼痛患者按常规三阶梯止痛处理治疗失败后,应再次分析疼痛原因,根据不同类型疼痛给予对症处理,不应一味增加止痛药剂量。
OBJECTIVE: To analyze the causes of pain and further treatment methods in patients with severe cancer who failed the three-step analgesic treatment. Methods: The patients with severe pain who failed in the conventional three-step treatment from September 2012 to April 2015 were collected and the causes of pain were analyzed again. The pain was further evaluated to evaluate the effect of pain control. Results: A total of 28 patients with refractory pain were enrolled in this study. The pain causes were analyzed for further treatment. Pain was relieved completely in 14 cases, partially relieved in 9 cases and slightly relieved in 5 cases. CONCLUSION: After the treatment of severe pain patients fails the conventional three-step analgesic treatment, the cause of the pain should be analyzed again and the symptomatic treatment should be given according to the different types of pain. Analgesic dosage should not be increased blindly.
