目的探讨高血压病肾内动脉血流参数变化的临床意义。方法将85例高血压病患者按高血压诊断分级标准分为3组,采用美国Acuson-Aspen彩色多普勒能量图(CDEI),检测其肾内动脉血流分布情况及相关参数,并与40例正常对照组进行比较。结果A组(28例)段间动脉收缩期峰值血流速度(Vmax)、舒张末期最低血流速度(Vmin)、阻力指数(RI)与正常对照组比较,差异均无统计学意义(P>0.05);B组(36例)和C组(21例)以上各参数均明显高于对照组(P<0.01),随肾损害程度的加重,肾彩色血流显像逐渐减少,阻力指数(RI)及脉动指数(PI)增高。结论肾动脉血流参数可以作为判断高血压病肾血管损害程度的主要依据之一。
Objective To investigate the clinical significance of changes of intrarenal arterial blood flow parameters in patients with essential hypertension. Methods Eighty-five hypertensive patients were divided into three groups according to the diagnosis and classification of hypertension. The Acuson-Aspen color Doppler energy map (CDEI) was used to detect the distribution of intrarenal arterial blood flow and related parameters. Cases of normal control group for comparison. Results The peak systolic velocity Vmax, Vmin and RI of group A (n = 28) were not significantly different from those of normal control group (p> 0.05). The parameters in group B (36 cases) and group C (21 cases) were significantly higher than those in control group (P <0.01). With the aggravation of renal injury, renal color flow imaging gradually decreased, and the resistance index RI) and pulsatility index (PI) increased. Conclusion Renal artery blood flow parameters can be used as the main basis for judging the degree of renal vascular damage in hypertensive patients.
