目的研究乙醛脱氢酶1(ALDH1)、低氧诱导因子1α(HIF-1α)和血管内皮生长因子(VEGF)在喉鳞状细胞癌(LSCC)组织中的表达及意义。方法采用原位杂交实验检测120例LSCC和60例对照喉组织中ALDH1、HIF-1α和VEGF mRNA表达,应用免疫组化Elivision法检测上述因子的蛋白表达情况,统计分析各因子表达与相关临床病理特征和预后的关系。结果 ALDH1、HIF-1α和VEGF mRNA在LSCC中的阳性表达率分别为57.5%(69/120)、73.3%(88/120)和56.7%(68/120),三者在LSCC中的蛋白阳性表达率分别为55.8%(67/120)、70.8%(85/120)和55.0%(66/120),与对照组比较差异均有统计学意义(P<0.05)。随着LSCC患者临床分期的增高及淋巴结转移的出现,ALDH1和VEGF蛋白的阳性表达率越高,差异有统计学意义(P<0.05)。HIF-1α蛋白的阳性表达与患者的TNM分期、组织分化程度、淋巴结是否转移有关(P<0.05)。三者的表达之间存在一定的正相关性(P<0.05)。ALDH1、HIF-1α和VEGF三种因子中两者(或三者)联合表达阳性的患者淋巴结转移率高于只表达任一单个分子或两者均不表达(或三者不同时表达)的患者(P<0.05)。三种因子表达阳性患者5年生存率均较阴性表达患者低(P均<0.05),三者联合表达的患者预后更差。结论 ALDH1、HIF-1α及VEGF在LSCC中表达上调,三者的联合检测对LSCC的发生、发展、侵袭转移和判断预后有一定意义。
Objective To investigate the expression and significance of ALDH1, HIF-1α and VEGF in laryngeal squamous cell carcinoma (LSCC). Methods The expressions of ALDH1, HIF-1α and VEGF mRNA in 120 cases of LSCC and 60 cases of control laryngeal tissue were detected by in situ hybridization. Results The positive expression rates of ALDH1, HIF-1α and VEGF mRNA in LSCC were 57.5% (69/120), 73.3% (88/120) and 56.7% (68/120), respectively. The positive expression rates of ALDH1, The expression rates were 55.8% (67/120), 70.8% (85/120) and 55.0% (66/120), respectively, which were significantly different from the control group (P <0.05). With the increase of clinical stage and lymph node metastasis in LSCC patients, the positive expression rate of ALDH1 and VEGF protein was higher, the difference was statistically significant (P <0.05). The positive expression of HIF-1αprotein correlated with TNM stage, histological differentiation and lymph node metastasis (P <0.05). There was a positive correlation between the three expressions (P <0.05). Patients with positive co-expression of ALDH1, HIF-1α, and VEGF in three (or all three) patients had higher rates of lymph node metastasis than patients who expressed neither single molecule nor did they both (or not all three) (P <0.05). The 5-year survival rate of patients with positive expression of three factors was lower than that of patients with negative expression (all P <0.05). The combined expression of these three factors had worse prognosis. Conclusion The expressions of ALDH1, HIF-1α and VEGF are up-regulated in LSCC. The combined detection of these three levels has some significance on the occurrence, development, invasion and metastasis of LSCC and the prognosis of LSCC.
