目的探讨共刺激分子B7-H3在人非小细胞肺癌细胞系和组织中的表达及其临床意义。方法利用逆转录-聚合酶链反应、流式细胞术及蛋白印迹技术检测B7-H3在肺癌细胞系中的表达;采用免疫组化SP法检测87例非小细胞肺癌、25例癌旁组织及24例肺部良性病变组织中B7-H3的表达并采用Image-Pro Plus软件测定每张切片的平均积分光密度。结果无论是mRNA水平还是蛋白水平,5个肺癌细胞系中均见B7-H3表达;B7-H3在非小细胞肺癌组织中的表达分别高于良性病变组织及癌旁组织(P<0.001);良性病变组织较正常肺组织表达高(P<0.001);B7-H3在分化程度好的非小细胞肺癌中的表达高于分化差者(P<0.05)。未发现B7-H3表达与患者年龄、性别、是否吸烟、组织学类型、肿块大小、有无淋巴结转移及有无远道转移的相关性(P>0.05)。本组B7-H3阳性表达率为67.8%,诊断灵敏度67.8%,特异度93.9%。Kaplan-Meier生存分析结果显示,B7-H3阳性表达患者的总体生存期明显优于阴性表达者(P<0.001);Cox比例风险模型分析显示,B7-H3阴性表达是非小细胞肺癌患者生存时间的独立预后因子。结论 B7-H3在非小细胞肺癌中过度表达,有助于非小细胞肺癌的诊断,并且可以作为肺癌患者的预后指标。
Objective To investigate the expression of costimulatory molecule B7-H3 in human non-small cell lung cancer cell lines and tissues and its clinical significance. Methods The expression of B7-H3 in lung cancer cell lines was detected by reverse transcription-polymerase chain reaction, flow cytometry and Western blotting. The expression of B7-H3 was detected by immunohistochemical SP method in 87 non-small cell lung cancer tissues, The expression of B7-H3 in 24 cases of benign pulmonary lesion and the average integral optical density of each section were measured by Image-Pro Plus software. Results The expression of B7-H3 was detected in all five lung cancer cell lines, both in mRNA and protein levels. The expression of B7-H3 in non-small cell lung cancer tissues was significantly higher than that in benign lesions and adjacent tissues (P <0.001), respectively. The expression of B7-H3 in well-differentiated non-small cell lung cancer was higher than that in poorly differentiated patients (P <0.05). No correlation was found between B7-H3 expression and age, gender, smoking status, histological type, tumor size, lymph node metastasis and distant metastasis (P> 0.05). The positive expression rate of B7-H3 in this group was 67.8%, the diagnostic sensitivity was 67.8% and the specificity was 93.9%. Kaplan-Meier survival analysis showed that the overall survival of patients with positive expression of B7-H3 was significantly better than that of negative expression (P <0.001). Cox proportional hazards model analysis showed that the negative expression of B7-H3 was the survival time of patients with non-small cell lung cancer Independent prognostic factor. Conclusion B7-H3 overexpression in non-small cell lung cancer is helpful for the diagnosis of non-small cell lung cancer and can be used as a prognostic indicator in patients with lung cancer.
