目的探讨冠心病(CHD)患者与甲状腺激素水平的关系及其临床意义。方法采用化学发光免疫分析法测定138例临床确诊的CHD患者及58例正常对照组的血清总三碘甲状腺原氨酸(TT3)、总甲状腺素(TT4)、游离T3(FT3)、游离T4(FT4)和促甲状腺素(TSH)水平。结果CHD组血清TT3水平为106.34±28.93ng/dL,TT4为7.60±2.35ug/dL,FT3为2.84±0.64pg/mL,FT4为1.25±0.35ng/dL,TSH为2.34±1.59ulU/mL。与正常对照组比较,CHD组血清TT3、TT4和FT3水平均显著低(P<0.05),而血清FT4和TSH水平变化不大(P>0.05)。结论CHD患者出现血清TT3、TT4和FT3水平下降,病情越严重,下降越显著。CHD时TT3、TT4变化的继发或伴发改变,是一种保护性反应。以TT3、TT4和FT3等指标可作为观察临床疗效和预后评估的依据之一。
Objective To investigate the relationship between thyroid hormone and thyroid hormone in patients with coronary heart disease (CHD) and its clinical significance. Methods Serum total triiodothyronine (TT3), total thyroxine (TT4), free T3 (FT3) and free T4 (total cholesterol) in 138 clinically diagnosed CHD patients and 58 normal controls were measured by chemiluminescence immunoassay FT4) and thyrotropin (TSH) levels. Results The serum levels of TT3 in CHD group were 106.34 ± 28.93ng / dL, TT4 was 7.60 ± 2.35ug / dL, FT3 was 2.84 ± 0.64pg / mL, FT4 was 1.25 ± 0.35ng / dL and TSH was 2.34 ± 1.59ulU / mL. The levels of TT3, TT4 and FT3 in CHD group were significantly lower than those in normal control group (P <0.05), while the levels of serum FT4 and TSH did not change much (P> 0.05). Conclusions Serum levels of TT3, TT4 and FT3 decreased in CHD patients, and the more serious the disease was, the more significant the decline was. CHD TT3, TT4 changes in secondary or associated changes, is a protective reaction. To TT3, TT4 and FT3 and other indicators can be used as a basis for observation of clinical efficacy and prognosis assessment.
