目的 :评价经颅彩色多普勒 (TCCS)诊断颅内动脉瘤的临床价值。方法 :采用 TCCS对 43例临床怀疑脑血管病患者进行检测 ,观察颅内主要动脉走行、形态 ,检测各项血流参数 ,盲法与 DSA结果进行对比分析。结果 :TCCS诊断颅内动脉瘤灵敏度 71% ,特异度 83% ,准确度 79% ,假阳性率 17% ,假阴性率 2 9% ,阳性预告值 6 7% ,阴性预告值 86 % ,阳性似然比 4.2 ,阴性似然比 0 .35 ,约登指数 0 .5 4。结论 :TCCS对颅内动脉瘤的诊断具有较高的实用价值 ,可作为早期筛检性诊断
Objective: To evaluate the clinical value of transcranial color Doppler (TCCS) in the diagnosis of intracranial aneurysms. Methods: Forty-three patients with clinically suspected cerebrovascular disease were examined by TCCS. The intracranial arterial walking and morphology were observed. The blood flow parameters were measured. The results of blinded and DSA were compared. Results: TCCS had a sensitivity of 71%, a specificity of 83%, a specificity of 79%, a false positive rate of 17%, a false negative rate of 29%, a positive predictive value of 6.7% and a negative predictive value of 86% However, compared with 4.2, the negative likelihood ratio of 0.35, the Youden index of 0.54. Conclusion: TCCS has high practical value for the diagnosis of intracranial aneurysms, which can be used as early screening diagnosis
