目的分析门诊西药房退药原因,探索减少退药的措施。方法收集2010年1月至6月门诊西药房1164张退药单进行统计分析。结果退药原因主要有医师(医患缺少沟通,计算机操作错误,更改治疗方案)、患者(检查项目取消,入院、转院,依从性差拒绝用药)、药房供应短缺、收费错误、药物不良反应等。结论实施规范退药后半年,退药减少了300例,初步取得成效。
Objective To analyze the reason of retreat in outpatient western pharmacy and explore ways to reduce withdrawal. Methods A total of 1,164 prescriptions for withdrawal from western medicine pharmacies in January 2010 to June were collected for statistical analysis. Results The main reasons for the withdrawal were physicians (lack of communication between doctors and patients, mistakes in computer operation, change of treatment plan), patients (examination items canceled, hospital admission, transfer and poor compliance), short supply of pharmacy, wrong charging and adverse drug reactions. Conclusion After the implementation of the code of six months after withdrawal, withdrawal reduced by 300 cases, the initial success.
