目的探讨乳腺小肿块超声BI-RADS指标特征与弹性超声结合对早期乳腺癌诊断的影响。方法对手术病理证实的153个最大径≤10mm的乳腺小肿块的超声BI-RADS征象进行分类,探讨各超声征象和病理之间的关系。并对其中128个的弹性图像以三分法进行评分,用两种方法对BI-RADS分类进行调整,比较两种方法的诊断效能。结果常规超声BI-RADS多项指标特征在乳腺小肿块良恶性间的差异有统计学意义(P<0.05)。弹性对乳腺小肿块的定性诊断灵敏度为89.9%、特异度为77.8%、准确率为85.6%。根据超声弹性对BI-RADS分类进行调整,两种调整方法结果显示方法一较方法二诊断效能更高。结论常规超声BI-RADS分类结合弹性超声可提高对乳腺小肿块的诊断能力。
Objective To investigate the diagnostic value of BI-RADS combined with elastic ultrasound in the diagnosis of early breast cancer. Methods The clinical manifestations of 153 small breasts with a maximum diameter ≤10 mm were classified by ultrasound BI-RADS, and the relationship between each ultrasound sign and pathology was explored. The elastic images of 128 were scored by trisection. BI-RADS classification was adjusted by two methods, and the diagnostic efficiency of the two methods was compared. Results There were significant differences in the features of multiple ultrasound biopsies between benign and malignant breast masses (P <0.05). The sensitivity of qualitative diagnosis of small breast mass was 89.9%, specificity was 77.8% and accuracy was 85.6%. The BI-RADS classification was adjusted according to the elasticity of ultrasound. The results of the two adjustment methods show that the method one is more effective than the second method in diagnosis. Conclusion Conventional ultrasound BI-RADS classification combined with elastic ultrasound can improve the diagnosis of small breast masses.
