1988年1月13日,河北衡水市某化肥厂,一煤气发生炉在加水时突然爆炸,当场炸死2人,重伤2人,216m~2锅炉房全部摧毁,直接经济损失5万多元。该煤气发生炉的水夹套等设备是该厂自行焊接制造的,于1982年安装运行,1987年7月停炉大修一次,更换了蜗轮、蜗杆、立轴、灰仓等部件,大修后工作正常。事故后对现场进行调查发现:炉底盘断裂,炉体倾倒,炉体焊缝全部撕开,水夹套内壁12＃钢板有5处向内鼓包,上下封头环形焊接部撕裂变形,并有过热过烧的痕迹,炉内夹套汽包、废热锅炉汽包,制汽分汽包
