目的探讨CT神经导航在经鼻蝶内镜下垂体瘤切除术的应用。方法在35例经鼻蝶内镜下垂体瘤切除术中应用神经导航技术,术前行CT扫描,在导航系统工作站中进行图像三维重建及制定手术计划,术中应用导航指导手术操作。结果35例导航手术均能精确定位,术中导航误差在1.5 mm以内,术后1~3个月复查MRI扫描示肿瘤全切24例、次全切10例、部分切3例。术中无正常组织损伤,术后无严重并发症。结论神经导航在经蝶窦入路内镜下垂体腺瘤切除术中起有效的辅助作用,定位准确、安全性高,有利于提高肿瘤切除率,减少并发症。
Objective To explore the application of CT nerve navigation in transsphenoidal pituitary tumor resection. Methods 35 cases of nasal endoscopic pituitary tumor resection using nerve navigation technology, preoperative CT scan in the navigation system workstations for three-dimensional reconstruction of the image and the development of surgical planning, intraoperative use of navigation guide operation. Results 35 cases of navigational operation could be accurately located. The navigation error was less than 1.5 mm. The MRI scan of 1 to 3 months after operation showed 24 cases of complete tumor resection, 10 cases of subtotal and 3 cases of partial cut. No intraoperative normal tissue injury, postoperative no serious complications. Conclusions Nerve navigation plays an effective auxiliary role in endoscopic pituitary adenoma resection through transsphenoidal approach. It has the advantages of accurate localization and high safety, which is beneficial to improve tumor resection rate and reduce complications.
