目的探讨前列腺液(EPS)pH值测定在鉴别Ⅲ型炎症性前列腺炎(ⅢA)和Ⅲ弄非炎症性前列腺炎(ⅢB)型前列腺炎中的意义,并通过分析Ⅲ型前列腺炎治疗前后pH值的变化,探讨前列腺液pH值的临床应用价值。方法按美国国立卫生研究院(NIH)制定的诊断标准,门诊收集初诊ⅢA型患者31例及ⅢB型患者33例。按国际慢性前列腺炎症状评分(CPSI)标准对所有患者进行疼痛症状(CPSI-P)和排尿症状(CPSI-U)分别评分,同时精密试纸检测pH值,显微镜下前列腺液白细胞(WBC)计数。比较ⅢA和ⅢB型患者前列腺液pH值的差异,对前列腺液pH值、WBC计数及患者的CPSl进行相关性统计学分析。同时对所有患者常规口服药物治疗,8周后复查前列腺液pH值、WBC计数,再进行相关性统计分析。结果ⅢA型患者前列腺液pH平均值为7.4±0.3,ⅢB型患者为6.5±0.3,两者比较差异有统计学意义(P<0.05)。相关性统计分析显示,Ⅲ型前列腺炎随前列腺液WBC数量的增加,前列腺液pH值逐渐升高,WBC数量与前列腺液pH值呈正性相关(P<0.05),前列腺液WBC数量、pH值与患者的CPSI无显著相关性(P>0.05)。ⅢA型前列腺炎治疗前后前列腺液pH值、WBC计数变化明显,差异有统计学意义(P<0.05)。ⅢB型前列腺炎治疗前后前列腺液pH值、WBC数量变化不明显,差异无统计学意义(P>0.05)。结论①ⅢA和ⅢB型前列腺炎前列腺液pH值比较差异有统计学意义,可以用来鉴别诊断ⅢA和ⅢB型前列腺炎。②前列腺液pH值与WBC数量呈正性相关,对慢性前列腺炎的诊断分型和治疗有指导意义。③常规口服药物治疗,ⅢA型前列腺炎治疗效果好,ⅢB型前列腺炎治疗效果差。
OBJECTIVE: To investigate the significance of determination of the pH of prostatic fluid (EPS) in the differential diagnosis of type Ⅲ inflammatory prostatitis (ⅢA) and type Ⅲ non-inflammatory prostatitis (ⅢB) type prostatitis. By analyzing the value of pH value before and after the treatment of type Ⅲ prostatitis Changes in the clinical application of prostatic fluid pH value. Methods According to the diagnostic criteria developed by the National Institutes of Health (NIH), 31 patients with newly diagnosed type ⅢA and 33 type ⅢB patients were collected at outpatient clinic. According to the International Chronic Prostatitis Symptom Score (CPSI), all patients were evaluated for CPSI-P and urinary symptoms (CPSI-U). Meanwhile, the pH value of the test strip was measured and the white blood cell count (WBC) was counted under the microscope. The differences of pH value of prostatic fluid in patients with type ⅢA and ⅢB were compared. The correlation between the pH value of prostatic fluid, WBC count and CPSl of patients was statistically analyzed. At the same time routine oral medication for all patients, 8 weeks after the review of prostatic fluid pH, WBC count, and then statistical correlation analysis. Results The average pH value of prostatic fluid was 7.4 ± 0.3 in type ⅢA patients and 6.5 ± 0.3 in type ⅢB patients. The difference was statistically significant (P <0.05). Correlation statistical analysis showed that with the increase of the amount of prostatic fluid WBC, the Prostate pH value gradually increased, the number of WBC was positively correlated with the prostatic fluid pH (P <0.05), and the number of WBC, pH and There was no significant correlation between CPSI and patients (P> 0.05). The changes of pH value and WBC count of prostatic fluid in type ⅢA prostatitis before and after treatment showed significant difference (P <0.05). The pH value and the number of WBC in prostatic fluid before and after ⅢB prostatitis treatment did not change obviously, the difference was not statistically significant (P> 0.05). Conclusion ①The pH value of prostatic fluid inⅢA and ⅢB prostatitis has statistical significance, which can be used to differentiate type ⅢA and ⅢB prostatitis. ② Prostatic fluid pH value and the number of WBC positive correlation, the diagnosis of chronic prostatitis typing and treatment is instructive. ③ conventional oral medication, type Ⅲ A prostatitis treatment is good, Ⅲ B type prostatitis treatment is poor.
