目的探讨阿奇霉素联合喜炎平治疗小儿支原体肺炎的临床疗效。方法选取宜章县人民医院儿科2016年5—11月收治的支原体肺炎患儿80例,根据入院顺序分为对照组和观察组,各40例。患儿入院后均给予常规治疗,对照组患儿给予阿奇霉素治疗,观察组患儿在对照组基础上给予喜炎平治疗,两组患儿均持续治疗5 d,而后停药3 d再持续治疗3 d。比较两组患儿临床症状及体征消失时间,治疗前后肺功能指标[用力肺活量占预计值百分比(FVC%)、第1秒用力呼气容积占预计值百分比(FEV1%)、最高呼气流速(PEF)、用力呼气25%流速(MEF25)、用力呼气50%流速(MEF50)]、细胞炎性因子[白介素6(IL-6)、白介素8(IL-8)、肿瘤坏死因子α(TNF-α)]及不良反应发生情况。结果观察组患儿退热时间、肺部啰音消失时间、咳嗽消失时间及就诊时间短于对照组(P<0.05)。治疗前,两组患儿FVC%、FEV1%、PEF、MEF25、MEF50比较,差异无统计学意义(P>0.05);治疗后,观察组患儿FVC%、FEV1%、PEF、MEF25、MEF50高于对照组(P<0.05)。治疗前,两组患儿IL-6、IL-8、TNF-α水平比较,差异无统计学意义(P>0.05);治疗后,观察组患儿IL-6、IL-8、TNF-α水平低于对照组(P<0.05)。两组患儿不良反应发生率比较,差异无统计学意义(P>0.05)。结论阿奇霉素联合喜炎平治疗小儿支原体肺炎临床疗效确切,可有效促进患儿疾病恢复,改善肺功能,降低机体细胞炎性因子水平,且安全性好。
Objective To investigate the clinical efficacy of azithromycin combined with Xiyanping in treating children with mycoplasma pneumonia. Methods Eighty children with Mycoplasma pneumonia admitted in Yizhang County People’s Hospital from May to November in 2016 were divided into control group and observation group according to admission sequence, 40 cases in each group. Children admitted to hospital were given conventional treatment, the control group of children given azithromycin treatment group, children in the observation group on the basis of the control group were given Xiyanping treatment, two groups of children were treated for 5 days, and then discontinued for 3 days and then continued treatment 3 d. The clinical symptoms and signs disappeared in both groups before and after treatment. The indexes of pulmonary function [FVC%, FEV1%, FEV1, PEF, MEF25, MEF50), cytokines (interleukin 6, interleukin 8, tumor necrosis factor alpha TNF-α)] and adverse reactions. Results In the observation group, the antipyretic time, the disappearance of pulmonary rales, the disappearance of cough and the duration of treatment were shorter in the observation group than in the control group (P <0.05). Before treatment, FVC%, FEV1%, PEF, MEF25 and MEF50 in two groups had no significant difference (P> 0.05). After treatment, FVC%, FEV1%, PEF, MEF25, In the control group (P <0.05). The levels of IL-6, IL-8 and TNF-α in the two groups were not significantly different before treatment (P> 0.05). After treatment, the levels of IL-6, IL- The level was lower than the control group (P <0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P> 0.05). Conclusion Azithromycin combined with Xiyanping treatment of children with mycoplasma pneumonia clinical curative effect can effectively promote the recovery of children with diseases, improve lung function, reduce the level of inflammatory cytokines, and good safety.
