目的:探讨经闭孔无张力尿道中段悬吊术(TVT-O)治疗女性压力性尿失禁(SUI)的临床疗效及应用前景。方法:回顾性分析2005年4月～2011年3月采用TVT-O手术治疗31例女性压力性或混合性尿失禁患者的临床资料:年龄43～79岁,平均55岁;病程3～15年。结果:所有患者手术顺利。手术时间17～40min,平均25min;术中出血量10～50ml,平均20ml;术后住院2～7d,平均3.5d。术中无并发症。2例术后仍有轻度急迫性尿失禁,经药物治疗后缓解。4例术后大腿根部疼痛,术后7～15天消失。31例随访2～71个月,平均36个月,无SUI症状复发及排尿困难。结论:采用TVT-O治疗女性SUI操作简单,效果可靠,康复快,远期疗效满意,是一种较为理想的手术方法。
Objective: To investigate the clinical effect and application prospect of transthoracic tension-free urethral suspension (TVT-O) in the treatment of female stress urinary incontinence (SUI). Methods: The clinical data of 31 female patients with stress or mixed urinary incontinence treated with TVT-O surgery from April 2005 to March 2011 were retrospectively analyzed. They were 43 to 79 years old with an average of 55 years and the course of disease was 3 to 15 years . Results: All patients operated smoothly. The operation time was 17-40 minutes with an average of 25 minutes. The intraoperative blood loss was 10-50ml with an average of 20ml. The patients were hospitalized for 2-7 days with an average of 3.5 days. No complications during surgery. Two patients still had mild urge incontinence after operation, which was relieved after drug treatment. Four cases of postoperative thigh pain, 7 to 15 days after the disappearance. 31 cases were followed up for 2 to 71 months with an average of 36 months without recurrence of SUI and dysuria. Conclusion: The treatment of female SUI with TVT-O is simple and effective, reliable, fast recovery and satisfactory long-term efficacy. It is an ideal surgical method.
