目的:探讨碘[131I]肿瘤细胞核人鼠嵌合单抗(131I-chTNT)注射液单疗程局部注射治疗恶性脑胶质瘤对甲状腺功能的影响与预防措施。方法:自2005年5月到2006年10月收治病理诊断的复发性脑胶质瘤患者随机分别进入对照组(TNT)和试验组(BCNU),其中对照组14例,试验组12例,分别使用卡莫司汀注射液和131I-chTNT治疗。观察甲状腺功能(TSH、T3、T4、FrT3、FrT4),治疗前、治疗后3个月和6个月的变化。结果:TNT组和BCNU组患者治疗前后TSH、T3、FrT3、FrT4、T4的测量结果,两组之间差异无统计学意义(P>0.05),但除FrT3之外,治疗前、治疗后3个月和治疗后6个月之间有显著性差异(P<0.001)。结论:131I-chTNT单疗程局部注射治疗恶性脑胶质瘤期间对甲状腺功能有明确影响。在使用131I-chTNT前必须有口服碘溶液,保护甲状腺功能。
Objective: To investigate the effects of 131I-chTNT injection on the thyroid function and its preventive measures by local injection in treating malignant glioma. Methods: Patients with recurrent gliomas who were diagnosed pathologically from May 2005 to October 2006 were randomly divided into control group (TNT) and experimental group (BCNU), of which 14 were in control group and 12 in experimental group, respectively Carmustine injection and 131I-chTNT treatment. The changes of thyroid function (TSH, T3, T4, FrT3, FrT4), before treatment, 3 months and 6 months after treatment were observed. Results: The results of TSH, T3, FrT3, FrT4, T4 before and after treatment in TNT group and BCNU group showed no significant difference between the two groups (P> 0.05) There was a significant difference between months and 6 months after treatment (P <0.001). CONCLUSIONS: 131I-chTNT has a clear effect on thyroid function during a single course of local injection of malignant glioma. Before using 131I-chTNT oral iodine solution must be to protect the thyroid function.
