目的探讨多层螺旋CT(MSCT)在外伤性面瘫中的应用价值。方法用MSCT对41例外伤性面瘫的颞骨行高分辨率轴面扫描。用Ph ilipsMx8000MSCT工作站,行岩锥感兴趣区的多平面重建(MPR)及面神经管的曲面重建(CPR)。观察骨折类型,骨折线的走行,中耳腔、乳突积血及外耳、中耳、内耳和面神经管的受累情况。结果41例患者中,39例颞骨有不同部位、不同类型的骨折,其中岩骨纵行骨折34例,横行骨折2例,混合型2例,1例岩骨无骨折,但颞骨鳞部骨折,2例未发现骨折。其中2例为双侧岩骨骨折。面神经管的CPR及MPR显示21例患者面神经管受累,9例可见骨碎片。结论MSCT扫描及重建技术能清晰显示面神经管的全貌及其损伤,对外伤性面瘫的诊断有重要意义。
Objective To investigate the value of multi-slice spiral CT (MSCT) in traumatic facial paralysis. Methods 41 cases of traumatic facial paralysis were scanned by MSCT with high resolution axial plane. Using PhilipsMx8000MSCT workstation, multi-plane reconstruction (MPR) of rock cone region of interest and surface reconstruction of facial nerve canal (CPR) were performed. The fracture type, the fracture line, the middle ear cavity, mastoid hemorrhage and the involvement of the external ear, middle ear, inner ear and facial nerve canal were observed. Results Among the 41 patients, 39 cases had different parts of the temporal bone and different types of fractures. There were 34 cases of osteochondral longitudinal fracture, 2 cases of transverse fracture, 2 cases of mixed type and 1 case of petrous bone without fracture. However, No fracture was found in 2 cases. Two of them were bilateral petrous bone fractures. Facial nerve tube CPR and MPR showed facial nerve tube involvement in 21 patients, bone fragments seen in 9 cases. Conclusion MSCT scanning and reconstruction technology can clearly show the whole picture of facial nerve tube and its damage, which is of great significance for the diagnosis of traumatic facial paralysis.
