目的探讨心包积液患者心包液中降钙素基因相关肽(CGRP)、内皮素(ET-1)与积液量、心功能的关系。方法35例心包积液患者按NYHA标准分为心功能Ⅲ、Ⅳ级,行超声心动图检查,计算积液量。行经皮穿刺心包腔内留置导管引流术时,取心包液及静脉血。正常对照组(血浆)30例。用放射免疫分析法测定心包液和血浆中CGRP、ET-1的浓度。结果心包液CGRP、ET-1的浓度显著高于血浆中的浓度(P<0.01),血浆中CGRP、ET-1含量较正常对照组明显升高(P<0.01),且心包液和血浆中CGRP、ET-1水平均与心功能相关(分别为P<0·01和P<0.05),心包液中CGRP、ET-1浓度与积液量呈正相关(分别为r=0.43、r=0.38,P<0.05)。结论心包积液患者心包液中CGRP、ET-1的浓度与心功能、积液量密切相关。
Objective To investigate the relationship between calcitonin gene-related peptide (CGRP), endothelin (ET-1) and fluid volume and cardiac function in patients with pericardial effusion. Methods 35 patients with pericardial effusion according to NYHA standards were divided into cardiac function Ⅲ, Ⅳ level, line echocardiography to calculate the volume of fluid. Percutaneous pericardial catheter indwelling drainage, pericardial fluid and venous blood. Normal control group (plasma) 30 cases. Radioimmunoassay was used to determine the concentrations of CGRP and ET-1 in pericardial fluid and plasma. Results The levels of CGRP and ET-1 in pericardial fluid were significantly higher than those in plasma (P <0.01), while the contents of CGRP and ET-1 in plasma were significantly higher than those in normal control group (P <0.01) The levels of CGRP and ET-1 were correlated with cardiac function (P <0.01 and P <0.05, respectively). The concentrations of CGRP and ET-1 in pericardial fluid were positively correlated with fluid volume (r = 0.43, r = , P <0.05). Conclusions The concentration of CGRP and ET-1 in pericardial fluid of patients with pericardial effusion is closely related to cardiac function and fluid volume.
