目的:观察肝动脉栓塞与经肝静脉逆行栓塞联合应用(THAE-RHVE)治疗原发性肝癌的疗效。资料和方法:8例患者进行THAE-RHVE。经肝动脉化疗栓塞时先用带囊导管阻断所属肝叶或段静脉,然后向肿瘤的供血动脉注入碘油-抗癌乳剂,最后注入明胶海绵碎粒;经肝静脉逆行栓塞时,在球囊导管完全阻断载瘤区肝静脉下逆行注入无水酒精-泛影葡胺(1∶1)混合剂。结果:THAE-RHVE技术均成功,无并发症。2~3周后CT复查肿瘤区碘油呈密实型充盈者6例,其中4例载瘤段门脉分支内有碘油沉积。术后1~2个月手术切除3例,2例肿瘤呈完全坏死,1例仅见少许退变的瘤细胞。5例随访5~18个月,1例复发。结论:THAE-RHVE后肿瘤区碘油呈密实型充盈者比例较高,肿瘤坏死较彻底,这一方法对限于一个肝叶/段的巨块型肝癌有良好疗效。
OBJECTIVE: To observe the efficacy of hepatic artery embolization and hepatic vein retrograde embolization (THAE-RHVE) in the treatment of primary liver cancer. Materials and Methods: THAE-RHVE was performed in 8 patients. In transcatheter arterial chemoembolization, the hepatic lobe or segment vein is blocked with a balloon catheter, then a lipiodol-anticancer emulsion is injected into the blood supply artery of the tumor, and finally the gelatin sponge is crushed; when the hepatic vein is retrogradely embolized, the ball is The balloon catheter completely blocked the hepatic vein under the tumor-bearing area and was retrogradely injected with a mixture of absolute alcohol-diatrizoate meglumine (1:1). Results: The THAE-RHVE technique was successful without complications. After 2 to 3 weeks, 6 cases of liposuction were found in the CT tumor area, and 4 of them had lipiodol deposition in the portal vein. Three cases were excised 1 to 2 months after surgery. The tumors were completely necrotic in 2 cases, and only a few degenerate tumor cells were seen in 1 case. Five patients were followed up for 5 to 18 months. One patient had recurrence. CONCLUSIONS: After THAE-RHVE, the proportion of iodized oil in the tumor area was dense and the tumor necrosis was relatively complete. This method has good curative effect on a hepatocellular carcinoma limited to one hepatic lobe/segment.
