目的 利用计算机辅助技术为髋臼发育不良全髋关节置换术中髋臼重建提供一种准确定位的手术方法.方法 对单侧髋臼发育不良患者进行薄层CT扫描,获取骨盆和双下肢二维图像数据,将数据导入医学图像处理软件处理,获得骨盆及双下肢三维解剖学模型.再将三维建模的数据导入逆向工程软件,建立健侧髋臼的镜像体,通过镜像体的髋臼准确定位患侧髋臼的解剖位置,模拟髋臼假体的植入.设计髋臼手术导航模板,导航模板中心确定为外翻45°、前倾15°,建立与患侧髋臼解剖学形态相匹配的导航模板.通过快速成型制作导航模板并应用于临床.结果 髋臼重建手术导航模板具有良好的匹配性,术中与髋臼骨面完全贴服.术中按照导航模板的指引,快速地确定了真臼的位置,按导航模板确定的方向及角度打磨髋臼,并安置了术前模拟置入直径的髋臼假体.假体与打磨好的髋臼窝完全匹配.术后X线片提示髋臼假体位于真臼位置,与周围骨床结合紧密.结论 手术导航模板的构建为髋臼发育不良全髋关节置换髋臼重建的精确定位、定向提供一种新的个性化手术方案.“,”Objective To provide a new method in precise reconstruction of acetabulum by computer-aided technique in total hip replacement(THA)for developmental dysplasia of the hip(DDH).Methods The patient with DDH was examined by lamellar CT.Data of pelvis and lower limb were obtained.The data were imported into medical image software.Three dimensional(3D)model was reconstructed.The data of 3D model were imported into reverse engineering software to establish enantiomorph of the acetabulum on the uninjured side.Anatomical position of the acetabulum on the injured side was located by the enantiomorph.Acetabulum prosthesis replacement was simulated.Navigation template was designed according to the anatomic features of the acetabulum on the injured side,which would be exactly manufacted by rapid prototyping and was applied in the patient.Results The navigation template was found to be highly accurate.The navigation template was sterilized and used intraoperatively to assist with surgical navigation and the replacement of prosthesis of acetabulum.The prosthesis was matched to the acetabulum.Conclusion The navigation template may be a useful method in the replacement of prosthesis of acetabulum in patient with DDH,by which individual operation project could be provided.
