目的探讨TCT、阴道镜、组织活检和HPV-DNA检测对于防治宫颈癌前病变的价值。方法从我院2016年1月—9月行宫颈疾病筛查者中选取38例同时进行TCT检查、阴道镜检查和HPV-DNA检查的患者,对患者中出现一项或者一项以上的异常者,进行病理活检。结果 38例患者中,经过TCT检测,37例呈阳性,其中有1例鳞状细胞癌(SCC)。患者中有3例高度鳞状上皮内病变(HSIL),包括1例CIN1、1例CIN2、1例CIN3。患者中有9例低度鳞状上皮内病变(LSIL),包括5例CIN1、2例CIN2、2例CIN3。患者中有24例非典型鳞状细胞(ASCUS),12例CIN1、8例CIN2、4例CIN3。有1例浸润癌患者。患者经过HPV-DNA检测,阳性35例,18例CIN1、10例CIN2、6例CIN3,1例浸润癌。经阴道镜检测,阳性32例,17例CIN1、9例CIN2、5例CIN3。经过TCT阳性病理组织活检,有CIN1有17例,CIN2有9例,CIN3有5例,1例浸润癌。患者通过TCT、阴道镜、组织活检和HPV-DNA检测,CIN1、CIN2、CIN3准确率比较高。结论 TCT、阴道镜、组织活检和HPV-DNA检测具有良好的诊断作用,能够为防治宫颈癌前病变提供准确的依据。
Objective To investigate the value of TCT, colposcopy, tissue biopsy and HPV-DNA testing in the prevention and treatment of cervical precancerous lesions. Methods From January to September 2016 in our hospital, 38 cases of cervical disease screening were selected for TCT, colposcopy and HPV-DNA examination in patients with one or more abnormalities in the patient , Biopsy. Results Of the 38 patients, 37 were positive after TCT test, of which 1 had squamous cell carcinoma (SCC). Three of the patients had high-grade squamous intraepithelial lesions (HSIL), including one CIN1, one CIN2, and one CIN3. There were 9 cases of low grade squamous intraepithelial lesion (LSIL), including 5 cases of CIN1, 2 cases of CIN2 and 2 cases of CIN3. There were 24 cases of atypical squamous cells (ASCUS), 12 cases of CIN1, 8 cases of CIN2, 4 cases of CIN3. One case of invasive cancer patients. Patients after HPV-DNA test, positive in 35 cases, 18 cases of CIN1, 10 cases of CIN2, 6 cases of CIN3, 1 case of invasive carcinoma. Colposcopy, positive in 32 cases, 17 cases of CIN1, 9 cases of CIN2, 5 cases of CIN3. After TCT-positive biopsy, CIN1 in 17 cases, CIN2 in 9 cases, CIN3 in 5 cases, 1 case of invasive carcinoma. Patients through the TCT, colposcopy, biopsy and HPV-DNA testing, CIN1, CIN2, CIN3 accuracy is relatively high. Conclusion TCT, colposcopy, biopsy and HPV-DNA test have a good diagnostic value, which can provide accurate evidence for the prevention and treatment of cervical precancerous lesions.
